Severe Acute Pancreatitis

  • Associated with organ failure and/or local complications such as necrosis
  • Clinical manifestations
    • Obesity BMI > 30
    • Hemoconcentration (hematocrit > 44%)
    • Age > 70
  • Organ failurea
    • Shock
    • Pulmonary insufficiency (PO2 < 60)
    • Renal failure (CR > 2.0 mg%)
    • GI bleeding
  • ≥ 3 Ransom criteria (not fully utilizable until 48 h)
  • Apache II score > 8 (cumbersome)

aUsually declares itself shortly after onset.
